Many chronically depressed and treatment-resistant patients experience immediate relief from symptoms after taking small amounts of the drug ketamine. For a decade, scientists have been trying to explain the observation first made at Yale University.
Today, current evidence suggests that the pediatric anesthetic helps regenerate synaptic connections between brain cells damaged by stress and depression, according to a review of scientific research written by Yale School of Medicine researchers and published in the Oct. 5 issue of the journal Science.
Ketamine works on an entirely different type of neurotransmitter system than current antidepressants, which can take months to improve symptoms of depression and do not work at all for one out of every three patients. Understanding how ketamine works in the brain could lead to the development of an entirely new class of antidepressants, offering relief for tens of millions of people suffering from chronic depression.
“The rapid therapeutic response of ketamine in treatment-resistant patients is the biggest breakthrough in depression research in a half century,” said Ronald Duman, the Elizabeth Mears and House Jameson Professor of Psychiatry and Professor of Neurobiology.
Duman and George K. Aghajanian, also professor of psychiatry at Yale, are co-authors of the review.
Understanding how ketamine works is crucial because of the drug’s limitations. The improvement in symptoms, which are evident just hours after ketamine is administered, lasts only a week to 10 days. In large doses, ketamine can cause short-term symptoms of psychosis and is abused as the party drug “Special K.”
In their research, Duman and others show that in a series of steps ketamine triggers release of neurotransmitter glutamate, which in turn stimulates growth of synapses. Research at Yale has shown that damage of these synaptic connections caused by chronic stress is rapidly reversed by a single dose of ketamine.
The original link between ketamine and relief of depression was made at the Connecticut Mental Health Center in New Haven by John Krystal, chair of the department of psychiatry at Yale, and Dennis Charney, now dean of Mt. Sinai School of Medicine, who helped launch clinical trials of ketamine while at the National Institute of Mental Health.
Efforts to develop drugs that replicate the effects of ketamine have produced some promising results, but they do not act as quickly as ketamine. Researchers are investigating alternatives they hope can duplicate the efficacy and rapid response of ketamine.
A BUNGLING drug user mistakenly handed over bags of cocaine to trick or treaters, thinking he had given them sweets. A court heard the children had been out for Halloween with their POLICE OFFICER father when they were handed the Class A drug. Donald Junior Green only noticed his error when he fished into his pockets and fished out packets of Haribo — realising he'd given away £200 of cocaine. Oldham Magistrates' Court heard the three children, aged eight, six and five, knocked on Green's door with their off-duty father. The hearing was told that Green, 23, dropped the drugs into the kids' goody bags. He then went back inside the house put his hands in his pockets to get out his drugs and instead pulled out the sweets. Green immediately realised what had happened and went on foot, then by car, scouring the streets of Oldham to find the youngsters. But their father, PC Simon Fowell, had taken his children home and, as they emptied their goody bags to share the spoils, he spotted the drugs — and made a swift call to his on-duty colleagues. Green, who had never been in trouble with police before and worked and went to college, wiped his eyes with his hands as he stood in the dock and admitted a single charge of possessing a Class A drug on October 31. Green, of Oldham, will be sentenced later today after the preparation of a probation report. Sean Brady, prosecuting, told the court Green was at his girlfriend’s house on Halloween night when the incident happened. The defendant had earlier that day bought £200 worth of cocaine, divided it up into snap bags and put it in his trouser pocket. His girlfriend had also bought some small bags of Haribo sweets which they had been handing out to trick-or-treating youngsters that evening. When PC Fowell’s children approached, Green put his hand in his trouser pocket, pulled out what he thought were Haribo sweets and dropped them in the goody bags they were holding. Mr Brady told the court: “Mr Fowell and the children thanked the defendant. That was their last call of the evening and they decided to go home - the children were soaked to their skin.” Back home the children went up to their bedroom to share out the sweets when their father popped in and spotted the “treat” from Green. “He asked his daughter where this item came from. She said it was from the last call,” Mr Brady told the court. Meanwhile, Green had realised his mistake. Mr Brady added: “Once he’s gone back inside and put his hand in his pocket again and instead has pulled out a pack of Haribo and immediately realised what he had done. “He then drove around the area trying to locate the children.” When Green answered the door to police who arrived to arrest him. he told them: “I know exactly why you are here. I knew you were coming.” Mr Brady said there was a “wry” side to the case but the consequences could have been very serious. “Had dad not have gone in and recognised what it was, this could have been a very serious case.” Steven Sullivan, defending, said Green had initially acted out of “public spirit” by giving out sweets - but that had gone badly wrong.
